First Visit
At your initial appointment, we will perform a complete oral health examination that will last approximately one hour. You may download the patient information and health history forms from this website and fill them out prior to your appointment. If you wish to fill them out at your appointment, please arrive approximately 20 minutes prior to your scheduled appointment time to do so.
